It is often missed as ‘viral fever’. An eschar in a skin fold plus a sick febrile child in Uttarakhand should make us think of scrub and treat on time.

We take a full history — onset, feeding, fever, sleep, school and any earlier reports.
Growth, chest, tummy, skin and development are checked. Tests are advised only if they change care.
A clear home plan is given, with a review so we know the child is settling.
Come in for fever beyond three days after outdoor play, especially with a black scab or severe headache.
